We collect information in two distinct areas: (1) Registration fields; and (2) Survey fields. The information we collect in the Registration areas is essential and includes:

  • First and last name
  • Email address

We also collect information that you voluntarily provide to us through responses to surveys in the training and subsequent to the training. This includes:

  • Discomfort level and frequency for various areas of the body

If you prefer not to report risk factors, you may either select “No” to bypass a specific question or click on the “Submit” button twice to move past the entire survey. If you prefer not to indicate your level of discomfort, you may simply click on the “Advance” button to move past this screen.
